You need to have finished the Stormcloak quest line, talk to Maramal about marriage and wear an Amulet of Mara. Sadly, it’s not voiced, and thus far I couldn’t get the follower dialogue to work. But our fertile imaginations can do the rest. (update- with the latest version he does follow me- barrels of fun having Ulfric as your battle sidekick)
